Could you please confirm if 138.750 and 138.850 were recently in Use?
Believe, but must admit not sure anymore, heard both Chinooks and Cougars on 139.125

142.750 is 301 Sqn, Gilze Rijen

While you mentioned Deelen TWR as a Soesterberg frequency, the following freqs should be in your list as well...

RAPCON west 375.8750 MHz
Soesterberg arrival 373.4250 MHz
RAMONES 1974 - 1996
Talking Heads 1974 - 1991

I'm missing 139.2500. Also an a/a. And are you sure EHSB does have a/a freqs assigned to squadrons (since Volkel and Leeuwarden don't have)?

EDIT: and what can be heard on the marshaller freqs? Usually marshallers are on the ground (at least at civil airports), so it's strange that they use an a/a freq :P .

139.250 is not A/A. It is a sort of Safety freq for GLV areas

I thought 140.875 was some kind of a/a ops or inflight ops or wing ops.

Well I'd say let's start a Leeuwarden topic :P .
