On 23 March 2022, a 125th Fighter Squadron/138th Fighter Wing F-16 Fighting Falcon crashed in western Louisiana. The OK coded Fighting Falcon, which is normally based at the Oklahoma Air National Guard's Tulsa base near Tulsa International Airport (OK), came down in an uninhabited, wooded area of Beauregard Parish, near the Louisiana-Texas border south of Fort Polk.

The Tulsa Vipers pilot ejected safely and was taken to an Army base nearby for check ups. During Fiscal Year 2021, the USAF sustained three F-16 mishaps. According to the USAF Safety Center, on average three F-16s have been involved in mishaps each year for the past decade.
